At Community Care Adult Foster Care & In‑Home Services LLC, we are committed to providing compassionate, structured, and person‑centered residential and in‑home support services for adults who require supervision and specialized care.
We bring years of experience to the field. Since 2018, our sister company, Red Maple Place 1 LLC, has been operating successfully and providing high‑quality residential services for adults with chronic injuries and TBI.
This foundation of experience strengthens our ability to deliver stable, therapeutic, and supportive environments where individuals can work toward independence, stability, and improved quality of life.

COMMUNITY CARE HOUSING
COMMUNITY CARE Homes are highly individualized, staff intensive, 24-hour care programs, both functioning in a residential home-like setting.
CCAF is designed for clients who require supervision and support.

Our Vision Statement
We serve with social beliefs and collaborative behaviors. This is accomplished by modeling consistency and trustworthiness throughout the entire milieu.
Our ancillary interdisciplinary team develops and updates an Individual Plan of Service for each resident and is modified annually or more frequently to support the client’s needs.

Our Commitment to Quality & Resources
Community Care ensures that all necessary resources — including trained personnel, clinical oversight, and supportive services — are available to meet the needs of each person we serve.
Our interdisciplinary team develops and maintains an Individual Plan of Service (IPOS) for every resident. This plan is:
- Created upon admission
- Updated annually
- Modified more frequently when clinical needs change
The IPOS guides all treatment, support, and daily structure within the home, ensuring that each resident receives consistent, coordinated, and effective care.
