An interdisciplinary team develops and updates an Individual Plan of Service (IPOS) for each resident. Plans are reviewed annually or more frequently as needed to ensure services align with evolving needs and treatment goals.
We ensure that all necessary resources, including personnel, funding, supplies, and space, are available to provide the services offered in each program.

Community Care was founded in October 1979 to provide a supportive residential program for adults with chronic and severe mental illness.
